چکیده

The anthracyclines are effective and widely used antineoplastic agents. They are used to treat many types of malignancy including acute lymphoblastic leukaemia.' Their introduction has considerably improved survival, especially in childhood cancers. Signs of cardiac toxicity may develop during, soon after or many years after a course of treatment. Early toxicity is dependent principally on the cumulative anthracycline dose. Typically the patient presents with left ventricular or congestive heart failure and with clinical features of congestive cardiomyopathy. Less often an arrhythmia is the first manifestation of toxicity. The major differential diagnoses are malignant pericarditis, cardiac injury from concomitant radiotherapy, and coincidental heart muscle or coronary artery disease. The two most widely used and studied anthracyclines are daunorubicin and its C-14 derivative, doxorubicin (adriamycin). They are glycoside antibiotics that owe their antineoplastic action to a complex effect that results in inhibition of nucleic acid synthesis by binding to both strands of the DNA helix, preventing the normal function of RNA and DNA polymerases.

Heart repolarization changes after anthracycline therapy in the children with cancer

Background Anthracyclines are important components of many chemotherapeutic protocols. The present study aimed to evaluate the repolarization changes in electrocardiography (ECG) which may predict drug induced arrhythmia. Materials and Methods In this cross-sectional study, the recorded ECGs were assessed for QT dispersion (QTd), QT corrected dispersion (QTcd), T peak to Tend dispersion (TPe...

Comparison of safety and toxicity of liposomal doxorubicin vs. conventional anthracyclines: a meta-analysis

UNLABELLED BACKGROUND Liposomal formulations of anthracyclines appear to have favorable toxicity profile when compared with conventional anthracyclines in elderly, high risk cardiac patients and patients with prior use of anthracyclines. Randomized controlled trials have evaluated the efficacy and safety profile of liposomal formulations with conventional anthracyclines. Our aim is to evalua...

Anthracyclines: molecular advances and pharmacologic developments in antitumor activity and cardiotoxicity.

The clinical use of anthracyclines like doxorubicin and daunorubicin can be viewed as a sort of double-edged sword. On the one hand, anthracyclines play an undisputed key role in the treatment of many neoplastic diseases; on the other hand, chronic administration of anthracyclines induces cardiomyopathy and congestive heart failure usually refractory to common medications. Second-generation ana...

Evaluation of long term cardiotoxicity after epirubicin containing adjuvant chemotherapy and locoregional radiotherapy for breast cancer using various detection techniques.

Breast cancer patients who present with only locoregional lymphatic metastases have good life expectancy after treatment with surgery and adjuvant anthracyclinecontaining chemotherapy. Anthracyclines, however, can induce cardiomyopathy, and this may become clinically manifest as chronic heart failure many years after exposure. As the occurrence of this side effect is dependent on the cumulative...

Evaluation of Anthracycline Effects on NT-ProBNP Plasma Level in Children with Malignancy

Background: NT-proBNP is a marker that is released from ventricles in response to pressure and volume overload. Raised plasma level of NT-proBNP is seen in ventricular dysfunction, ventricular muscular mass reduction or ventricular ischemia. Anthracyclines are widely used in treatment of pediatric cancer but their use is associated with cardiotoxicity which increases mortality and morbidity. We...
